On June 1, 2022, as part of the rollout of an updated Person-Centered Planning Process, Home and Community Based Service (HCBS) providers will begin using a new service planning tool called the Service Implementation Plan ("SIP"). The SIP is used to document the HCBS provider's service delivery strategies for the requested waiver service, in order to support a Member's chosen goal(s)/ outcomes(s). The SIP will have fields to specify strategies for goal implementation that are specific, measurable, achievable, and have clear proposed timelines for how the service will met the member's goals. A training is now available which provides an overview of the SIP.
